Charting load is easy to underestimate because it never arrives as a block. It is four minutes here, a note you meant to finish, a photo still on your phone, a lot number you will write down after this next patient. By Thursday it is an evening, and by December it is a stack of visits where the product cannot be identified with certainty.

The last figure is the one to act on first, because untraceable visits are the finding that turns a routine inspection into a corrective action plan.
A chart finished at the chair takes less time than the same chart finished at eight in the evening, because the details are still in front of you. The carton with the lot number is in the sharps area, the units you injected are fresh, and the patient is there to sign. Recreated later, every one of those becomes a recall problem, and recall problems are where inaccuracies enter a legal record.
The practices that get charting under five minutes rarely do it by typing faster. They change the sequence: consent signed, photos shot and lot captured before the first needle, with only units and any reaction noted afterward. That reorder is what moves the work from evening to chairside.
The untraceable visit count is not theoretical. If a manufacturer issues a notice on a batch, or a patient reports a delayed nodule and you need to know which product went in, those records are the ones you cannot answer for. State boards treat missing product traceability as a documentation failure regardless of whether any harm occurred.
